Step 1
Secure and accomplish an Action Sheet Form from the Professional Regulation Commission (PRC) Customer Service Center (CSC).
Step 2
Submit the Action Sheet form in any processing window together with the following:
- Original and photo/xerox copies of Transcript of Records with Special Order and Date of Graduation
- With SCANNED PICTURES AND WITH REMARKS โFOR BOARD EXAMINATION PURPOSES ONLYโ .View Res.2004-2000 .(Graduates of government schools and institutions/programs accredited by recognized accredited agencies under the FAAP are exempted from SO [B]). Graduates of New Schools/degree Programs must submit School Recognition and/or Permit to operate.
- Original and photo/xerox copy of NSO-issued Birth Certificate (in NSO security paper; if NSO copy not very clear, bring copy from the Local Civil Registrar)
- Original and photo/xerox copies of Marriage Contract in NSO security paper (for married female only;if NSO copy not very clear, bring copy from local civil registrar).
- Four (4) passport size colored pictures in white background with complete name tag.
- Current Community Tax Certificate (Cedula)
- Other specific requirements as required by the Commission and/or Professional Regulatory Board.
Note: Upon approval of your application, secure the following from the processor
- Computerized Application form (CAF)
- Permanent Examination and Registration Record card (PERRC) (for first-timers)
- Notice of Admission (NOA)
Step 3
Pay your examination fee at the cashier windows and do not forget to get your Official Receipt
Payments:
- Baccalaureate Degree – P 900.00
- Non-Baccalaureate Degree – P600.00
- Removal – P450.00
Step 4
At the CSC pay for metered documentary stamps and mailing envelope with metered documentary stamp.
Step 5
Accomplish the CAF, PERRC, and NOA. Indicate the Official Receipt number, date, and amount on the CAF.
Step 6
Go to any issuance windows for final review of qualifications and documents and issuance of NOA, Applicant’s Stub, Program of Examination, Examinee’s Guide and Self-Instruction Sheet.
Step 7
You will be bringing the following on the first day of exam so make it sure to keep this
- Notice of Admission
- Official Receipt
- Applicant’s Stub
Step 8
Bring your Notice of Admission when you verify with PRC your school and building assignment which will be posted at the PRC premises 2 โ 3 working days before the examination date.

ask ko lang po kung hindi na po kailangan yung dating noa pag 3rd taker ka? nawawala po kasi yung noa ko dati hindi pa po ako makapagfile kasi hindi ko po lam kung kailangan pa yung last noa. Thank you po. God Bless.

Thank you for visiting. YOu need to bring your Previous NOA to file in taking the board again. In case you lost your NOA go to PRC and tell them. They will tell you what to do. thank you

You can ask an original birth certificate from NSO and if malayo ang sa inyo you can visit LBC.
Fill out ka lang ng form nila then pay about 300 pesos then you can have it deliver at your place or pick up then you will rceive 2 original compies of your NSO
Hello po, ask ko lang po. What if nakapag file na po kami, then that day is manila day po kase kaya Hindi po namin nakuha NOA namin, pero anytime daw po pwede kunin. Does it mean po na pwede parin po makuha yun kahit august or before exam? As long as nasamin po yung receipt. Or kailangang hanggang july 29 lang po dapat kunin although nakapag file na po kami? Where you able to take the exam without the OR?
In actual board exam you’ll be needing your O.R. number and it’s in your NOA. It is written there. No problem if the receipt was lost. ?
So okay lang po na walang official receipt? Di naman nila kinocollect? Sa case ko po kasi sa LBC ako nagbayad. I do still have my NOA though.

Paano po pag may mali sa surname? Ang transcript ko Po ay AGUA which is our real name, pero AQUA ang nasa NSO birth ko. Paano po yung may ganitong problema? salamat po

Good day po maam. Nabasa ko po na kelangan dalahin yung NOA kpag magfa-file ulit ang retaker, pano po kung nawala yung NOA sa previous exam.

Hi po. After ko po kasi mag-exam nawala ko yung NOA ko, nakapasa po ako then nalaman ko na requirement din yung NOA sa pre-registration. Kaya pumunta po ako ng PRC, tinanong ko kung ano gagawin. Basta alam po yung application number, okay na po yun. Yun lang naman daw po kailangan sa pre-pre-registration. Nasa online profile naman po, sa website ng PRC yung applicatiom number, kung di niyo po alam talaga ata wala sa online profile niyo po, pwede niyo po puntahan sa PRC at dun itanong.
